Invitations have been mailed to all of our Association’s ministers, ministers’ surviving spouses, and missionaries age 65 and over.
Please note when filling out the booking form below:
FOR AN INDIVIDUAL BOOKING:
- The first “name field” is for the Attendee.
- The second “name field” is for the person filling out the form based on the assumption that the person attending the event isn’t necessarily the person signing them up.
- If you are filling this out for yourself, just enter your name in both “fields.”
FOR TWO OR MORE PEOPLE:
- After choosing the number of spaces, the corresponding number of “Attendee Name” fields will appear. Please list the names of those attending in each “Attendee Name” field. If you are filling this out for yourself and a guest, just enter your name in the “Name” field.
